Fill out this confidential Report Form and someone will speak with you as soon as possible regarding protocols, next steps, and when you (Coach or student) might be able to return to play. PLEASE COMPLETE A SEPARATE FORM FOR EACH PERSON. If you have any questions, please contact the Compliance Manager Chad Coss at (612) 402-5611. If you are a Parent or Guardian reporting, we will share this info with your child's coach as well. We are required to report the cases to the MDH and the School District as well.
ONCE THE FORM IS COMPLETE - If the Admin has any questions, they will contact you. Any athlete or coach with a positive COVID result must not return to practice until 5 days after their positive results and may return to practice if they are no longer symptomatic. The BBC COVID protocols are not intended to be medical advice - please get in touch with your provider for any questions regarding COVID treatment.
